Providence is calling a MDS Registered Nurse (RN) Case Manager (Transitional Care Unit, Variable Shift, 1.0 FTE) to Providence Mount St. Vincent in Seattle, Washington. In this position you will:
Conduct a comprehensive admission assessment for new patients.
Initiate an individualized nursing care plan based on assessment data, and shares important information about patients with IDT members.
Oversees Resident Assessment Instrument (RAI)/MDS process for assigned patients, including: monitoring the assessment schedule in collaboration with Health Information staff; timely and accurate completion of Minimum Data Set (MDS) nursing assessments; and coordination of completion of other MDS sections with IDT members.
Required qualifications for this position include:
Maintains current RN licensure in Washington State
Cardiopulmonary Resuscitation certification within 30 days of hire
Food Handler's Permit within 90 days of hrie
2 years of Long Term Care, Transitional Care, or equivalent experience
Proficiency in technology
Preferred qualifications for this position include:
Bachelor's Degree in Nursing (BSN)
