The W. G. (Bill) Hefner VA Medical Center of Excellence in Geriatrics and Extended Care (GEC) located in Salisbury, NC is seeking an experienced full-time Board certified/Board eligible Geriatrician to join an academically affiliated group practice providing a full array of Geriatric, rehabilitation, palliative, hospice, community and home care to Veterans living in the North Carolina Piedmont area with a focus to maximize their function, independence and quality of life.
The incumbent will join two fellowship-trained Geriatricians and six nurse practitioners working in various venues including a 124-bed Carolina Pride Community Living Center (CLC), which is currently being renovated to include a new Hospice House, indoor Main Street and park area with a water fall and stream, 90% single bedrooms with private baths and a gymnasium to name just a few features associated with the provision of state-of-the-art Geriatrics and Hospice care. The Home-Based Primary Care Program is expanding with its major focus on providing primary care and some post-hospital care in the Veteran's home. The W. G. (Bill) Hefner VA Medical Center proudly served over 85,000 Veterans and 660,000 outpatient visits last fiscal year and is one of the fastest growing VAs in the country for the past several years.
TheGEC Service Line is committed to and responsible for ensuring quality outcomes of Veteran care, ready access to care and excellence in customer service to support the Medical Center and VHA mission to serve Veterans and their families. The incumbent will participate in direct Veteran care, work directly with mid-level providers, instruct and supervise students, medical residents and Geriatric Fellows associated with the Wake Forest School of Medicine, the Virginia Tech School of Osteopathic Medicine and other professional schools in the area. Other responsibilities include assessment of Veterans referred for admission to the CLC, performing comprehensive history and physical examinations, Geriatric assessments, medication reconciliations, develop interprofessional treatment plans of care, etc., to achieve optimal interdisciplinary expertise and services for the frail, older or disabled Veteran. In addition, the incumbent is expected to fully participate in performance improvement and continuous readiness activities to support ongoing excellence in Veteran care. The GEC staff is encouraged to participate in independent and/or collaborative investigative research. The W. G.
