People’s Health Center is seeking full-time employment of a Certified Diabetes Educator (CDE). The CDE works within the Chronic Condition Management program. The CDE is responsible for assessment, planning, and implementation of a customized educational program for our Diabetic population. Will provide Care Coordination for the multidisciplinary team through consultations and referrals to other health care professionals including: Providers, wellness coaches, exercise physiologists, and/or registered dieticians. Integrates patient assessments and health history information to develop a comprehensive, self-management care-plan for patients with chronic conditions. Will outreach to a population of high-risk patients as necessary to provide additional support and guidance to leverage patient engagement.
CDE certification, required, and may be a RD or a RN. The CDE will provide education and support to individuals and groups, according to their individual Scopes of Practice.
People’s Health Center is a federally qualified health center (FQHC). FQHC is a federal designation from the Bureau of Primary Health Care (BPHC).
FQHCs are either located in or serve a federally designated medically under-served area/population. All FQHCs must operate under a consumer Board of Directors governance structure and provide comprehensive primary health care, dental, and mental heal...th/substance abuse services to persons in all stages of the life cycle.
FQHCs provide their services to all persons according to their ability to pay. There are over 1,200 Community Health Centers across the country that provide vital primary care to over 18 million Americans with limited financial resources.
People's Health Center is proud to be the only FQHC serving the people of Lincoln-Lancaster County. People’s Health Center provides affordable, comprehensive, accessible, culturally appropriate, cost-effective primary health and dental care. PHC serves people of the Lincoln and Lancaster County, especially those individuals/families with limited resources or with other barriers to health care to improve their overall health status.
