The Manager of Clinical Pharmacy, Ambulatory Care supports the mission of MemorialCare Medical Foundation and the Pharmacy Department by assisting in the development, implementation and maintenance of ambulatory care services which includes but is not limited to pharmacist led ambulatory care services.
Essential Functions and Responsibilities of the Job
* Provides leadership/direction and manages/coordinates projects related to planning, development, implementation and evaluation of clinical pharmacy programs * Determines/obtains resources needed to support implementation of initiatives for clinical pharmacy services including but not limited to literature review, protocol development, data collection, outcomes measurement and analysis * Participates in provider meetings as required * Be compliant with pharmacy laws * Document patient care activities * Monitors drug therapy according to established protocols, policies and procedures * Serves as a mentor, provides guidance and manages the staff including but not limited to: clinical pharmacists, nurse practitioners, dietitians and medical assistants in the ambulatory care clinics (diabetes, anticoagulation, and post-discharge) * Serves as a drug information expert and consults with medical providers on optimal drug therapy regimens and drug therapy management and provides drug information when appropriate * Hire staff and perform performance reviews
* Prior management experience required (2-5 years minimum) * Experience with diabetes medication management and other chronic disease management (including anticoagulation and medication management for the geriatric population) * At least 5 years of postgraduate work as a health care provider with direct patient care responsibilities, aside from any dispensing functions * Formal training in clinical pharmacy or relevant experience in clinical pharmacy in a medical setting and ability to work within a multi-disciplinary team * Must possess effective verbal and written communication skills; experience in managed care; and demonstrated ability to exercise collaboration, negotiation and other interpersonal skills * Must demonstrate record of success in working with physicians and within multidisciplinary healthcare professions * Position requires comprehensive knowledge of pharmacology, medical terminology, pathologies, disease states, evidenced-based guidelines and related information to medical management plan * PC skills including Microsoft Word, PowerPoint, and Excel are required
* Doctor of Pharmacy Degree, from an accredited school of pharmacy * Active License to practice pharmacy by the California Board of Pharmacy * Successful completion of a clinical pharmacy residency program which conforms to the American Society of Health Systems Pharmacist standards required and certification such as Board Certified Pharmacotherapy Specialist, etc. preferred
