The Clinical Case Manager is concerned with and, under the supervision of the Director of Rehabilitation, responsible for the development, implementation and management of all clinical aspects of Centre for Neuro Skills. He/she is an integral part of the overall decision making process bridging both clinical and administrative matters involving both the clinic and residential sites. Work is full time during the weekdays (M-F, 7:30am - 4:30pm) and requires some on-call and overtime work.
For over 35 years, CNS has provided post-acute medical treatment, rehabilitation and disease management, and offers residential, inpatient, outpatient and assisted living programs. Locations include Bakersfield, Los Angeles, San Francisco, Dallas, and Fort Worth.
Our facilities have been honored many times by the prestigious Commission on Accreditation of Rehabilitation Facilities (CARF International) for excellence in therapeutic programs, staff education, patient treatment, and leadership. We also conduct research on brain injury and recovery, with a dedicated, in-house team of Ph.D. researchers who collaborate with major U.S. universities. Knowledge, Skills, and Abilities Required:
Master’s degree or professional licensure/credential in a related field required.
License by a therapy board of PT, OT, or SLP is preferred.
CCM preferred. Successful completion of CCM required upon meeting eligibility guidelines.
Minimum of 3 - 5 years’ experience in rehabilitation or related field.
Experience in which supervisory abilities have been demonstrated.
High level of accuracy and dependability in decision making, coordination of patient programs, documentation, and ability to evaluate situations as they occur.
