Supports the clinica, technological and practice management integration of primary care practices to PCMH (Patient Centered Medical Home) model. The PCMH is a health care setting that facilitates partnerships between individual patients, and their personal physicians, and when appropriate, the patient's family. Care is facilitated by registries, information technology, health information exchange and other means to assure the patients receive the indicated care when and where they need and want it in a culturally and inguistically appropriate manner. Position will support the sucessful change resulting in sustainable processes and cultivating an innovative program for improving primary care.
Bachelor's Degree in Health Care Administration, or equivalent training required or LVN
Strong analytical, data management and computer skills; Prefer two years' experience in the following areas: a. Microsoft Excel to develop spreadsheets, analyze and manipulate data and produce reports b. Data analysis and reporting tools (e.g. Microsoft Access, Excel, Meditech, HBI, HPM, Athena c. Developing and preparing a variety of reports and analyses d. Synthesizing, evaluating and interpreting information.
Excellent organizational, prioritization skills and interpersonal skills.
Possess skills for Systems,and Operation analysis
Deductive and Inductive Reasoning
Critical Thinking Skills
Strong IT background
Experience in project management
Experience in practice - clinical, practice management or other relevant experience
Experience with health care and managed care terms preferred.
