Part of multidisciplinary treatment team in developing a treatment plan for each patient and documenting services performed in the client record.
Oversee linkage and contact with family, guardian and state agencies including participation in completing necessary documents for Illinois Department of Children and Family Services, Illinois Department of Human Services, Illinois State Board of Education and other funding sources in support of ongoing treatment and comprehensive services.
Responsible for facilitating successful discharge and transition planning consistent with state agency policy and procedures.
Responsible for all case management activities involving travel including facilitating client transport to court appearances, planning meetings, placement visits and other activities necessary to treatment.
Assures coverage of scheduled activities such as educational and therapeutic groups.
Coordinates the development of a therapeutic milieu through an honest, supportive and structured atmosphere.
Keeps informed of community treatment resources from referrals to and from the program for patients and their families as evidenced by listing service for mental health, group home, assistance programs, 12-step programs and DCFS.
Participates in program planning (recommends curriculum, program changes, etc) with other staff and supervisor.
Develops referral resources and maintains regular contact.
Utilizes knowledge and skills related to crisis intervention and verbal de-escalation in order to provide a safe environment for patients, visitors and staff.
