The Nurse Practitioner is a licensed independent practitioner who is responsible for managing health problems and coordinating health care for the CHRISTUS Health Quality Care Alliance transition of care patients in accordance with State and Federal rules and regulations and the nursing standards of care (assessment of health status, diagnosis, development of plan of care and treatment, implementation of treatment plan, and evaluation of patient status).
Obtains complete medical history and physical data on patients.
Interprets and integrates data to determine appropriate diagnostic and therapeutic procedures as needed.
Performs a complete physical exam and records finding. Collaborates with providers in managing transition of care medical needs of patients.
Orders appropriate laboratory and diagnostic procedures
Synthesizes data to determine diagnosis and therapeutic plan utilizing principles of prevention.
Interviews and advises patients and their families regarding health and illness prevention. Recommends community resources to meet patient and family needs.
Instructs patient and family regarding medication and treatment instructions. Provides patient education.
Maintains and reviews patient records, charts and other pertinent information, post-tests and examination results.
Prepares medical records and charts for all patients treated by Associate, in a timely and accurate fashion and in conformity with CHRISTUS requirements, applicable standards of professional practice and the rules regulations and guidelines established by state or federal governmental authorities, including the Centers for Medicare and Medicaid Services.
Manages medical emergencies.
Practice using evidence-based practices; standardized protocols and order sets, appropriate utilization of resources, and coordination of care to prevent hospital readmissions.
Participates in care-related activities designed to improve quality, ensure compliance, utilization management, patient satisfaction, and support programmatic efficiencies.
Has a working knowledge of ICD10, TOC billing codes, CPT, coding and manages care, and be available for training as necessary.
Attends required meetings and participates in committees/pilot projects as requested.
Participates in professional development activities and maintains professional affiliations
Follows the CHRISTUS guidelines related to Health Insurance Portability and Accountability Act (HIPPA), designed to prevent or detect unauthorized disclosure of Protected Health Information (PHI)
Maintains strict confidentiality
Uses oral and written communication skills to effectively convey ideas in a clear, positive manner that is consistent with the CHRISTUS Mission
Maintain established CHRISTUS Trinity Mother Francis policies, procedures, objectives, quality assurance, safety, environmental and infection control

Associate will enter into collaborative agreement with Supervising Physician and work under his/her direct supervision to perform tasks as assigned.
Associate collaborates with primary care physician and integrates as a member of the patient's care team.
Performs other related work as required
Graduate of an accredited school of nursing,
Successful completion of an approved Nurse Practitioner program.
Basic Computer Knowledge
Ability to communicate effectively, both verbally and written.
Three years professional nursing experience in a clinic setting.
C. Licenses, Registrations, or Certifications
High school diploma or equivalent.
Valid State Registered Nurse license and Advanced Practice license certified by an accredited certification agency recognized by the state board of nursing.
