Sierra Vista Hospital, a 171-bed psychiatric care facility located in south Sacramento, is a full-service mental health facility and a center of support for patients and their families. Since 1986, we have offered our clinical expertise to those suffering from emotional, behavioral and addiction problems.
The Outpatient ClinicianÂ is assigned a caseload of patients. The responsibilities of this position include, but are not limited to: assessment, discharge planning, reporting during treatment planning, maintaining patient attendance to prescribed treatment, following-up to help ensure the safety of all patients, and ensuring documentation compliance. The Therapist coordinates the initiation of a Master Treatment Plan and updates that are consistent with the findings of initial assessments, ongoing assessments, treatment team meetings and other documented observations. The Therapist attends daily utilization review/clinical meetings and ensures that the behavioral treatment provided to the patient is consistent with what is communicated to and understood by the Utilization Review Specialist, management and other team members and that this information is consistently documented in the chart across disciplines. The Therapist facilitates groups and 1:1 meetings using an evidence-based treatment consistent with research findings. The Therapist exercises sound judgment and clinical excellence, sustains therapeutic alliance and patient satisfaction. The Therapist will be cross-trained and assigned, as determined by department need, in the Intake and Utilization Review functions.
