Summary: This position will act as Coach/Educator for Patients, their families and other health care personnel to ensure that all the parties providing services, understand and engage in the “Plan of Care.” This position will establish Care Plans, which include patient referral or admission, medication doses, administering therapies and treatments, evaluation of results and a summation of the plan's effectiveness.
This position will be based out of our skilled nursing facility, Lieberman Center for Health and Rehabilitation in Skokie, IL.
Adheres to the scope of practice for Registered Professional Nurse per state regulatory guidelines
Demonstrates appropriate judgment skills to be able to make independent clinical decisions in routine patient care matters.
Promote the level of clinical expertise required of staff to provide safe, high quality nursing care.
Remains flexible and responsive when changes occur in patient activity and workload.
Fosters an environment for open communication and collaborative practice.
Evaluates care plans and update as needed.
Demonstrates knowledge of therapeutic action, side effects and interaction of medications
Graduate from an accredited School of Nursing-BSN
Must be currently licensed to practice as a Registered Nurse in Illinois.
One or more years experience in geriatric or acute care.
Flexible schedule required.
Computer skills including knowledge of MS Office.
