The Social Worker/Therapist plans, provides, coordinates, documents, and evaluates direct and indirect social services to patients in accordance with established methods and procedures of the hospital.Â These services include but are not limited to the following: individual therapy, group therapy, family therapy, case management, treatment planning, rounding with clinical staff, attending/conducting unit and hospital meeting, discharge/care planning and follow-up, resource development/assistance, and serving as a member on various committees.Â The Social Worker/Therapist further contributes to Social Services and the unit on which he/she works through support of philosophy and objectives, educational efforts and Quality Management efforts.
