We are recruiting for a Placement Liaison Manager to join Lacuna Health in Louisville, KY.
Oversee all aspects of Discharge Placement Solutions at the Liaison level. This includes responding to escalations and resolutions within established service levels, as well as managing a team of liaisons.
Ability to interface with Lacuna Health and Customer operations, as well as on the ground teams to ensure customer service levels are being reached and maintained
Conduct ongoing customer satisfaction surveys. Generate and/or circulate weekly and monthly statistical reporting in areas such as: customer satisfaction, nurse and team member productivity, issue identification and trending as well as escalation to senior management of any recognized customer need.
Closes gaps in communication and escalates issues at appropriate time and through appropriate channels.
Acts as a decision point for critical points of the Lacuna Placement Team and facility workflow.
Required Travel : 70%
Communication - Establishes a consistent rapport with both internal facility staff and Lacuna Placement Team. Closes gaps in communication and escalates issues at appropriate time and through appropriate channels. Reports and relays information within designated timeframes to both facility staff and Lacuna Placement Team. Ensures consistency and uniformity in process from patient to patient as closely as possible.
Clinical Oversight - Follows patients clinically and assists in identification of barriers to discharge through IDT meetings and EMR documentation, when alerted by the Lacuna Health Liaison. Ensures patient is receiving patient centered care and tracks progress toward anticipated date of discharge, identifying barriers along the way.
IDT Meetings - Available for any escalations stemming from IDT team meetings and Care Transitions meetings at facility level. Participates in scheduled Lacuna Placement Team meetings. Works closely with Lacuna Health Liaison, Area Directors of Case Manager and District Director of Case Management at facility to ensure appropriate information is conveyed to patient and family members.
Escalation - Establishes professional and appropriate repertoire with facility team and Lacuna Placement Team to create escalation network. Escalates designated issues at appropriate time and to appropriate personnel. Identifies and addresses issues through to resolution.
Leadership - Maintains a positive environment and encourages questions, clarifications, and an open approach to the Placement Model. Possessed ability to encourage a collaborative work environment between both teams for a smooth patient experience and transition.
Must have excellent interpersonal, communication, and organizational skills.
Ability to escalate issues and see through to resolution point.
Ability to handle large patient volume with scheduled tasks and deadlines.
Proficient presentation, facilitation, and public speaking skills for IDT meetings.
Proficient user knowledge of Windows Office programs (Word, Excel, PowerPoint), and the ability to learn specialized computer applications (Protouch, Ensocare, Point Click Care, HPAS, Meditech) that are specific to handling job requirements.
Ability to develop professional working relationships and communicate effectively as a team leader
Ability to plan, organize and prioritize multiple tasks that need to be completed on a daily basis.
Ability to maintain confidentiality.
Ability to work independently. Must be self-motivated and goal oriented.
Ability to communicate effectively, both orally and in writing.
Ability to maintain regular attendance.

Associates Degree or higher in Nursing.
Bachelor's degree preferred.
Must have an active RN license.
Must have a valid driver's license.
5 years of Healthcare experience.
3 years of experience in Case Management highly preferred.
