The Transitional Coordinator assists with preparation of discharge and planning for transition to home from the facility. This position is responsible for all facets of the transitional component of the program as well as direct services delivery to the youth. Okaloosa Youth Development Center (OYDC) is a high-risk program that offers Mental Health Overlay Services (MHOS) to youth, ages 13 to 18, with moderate- to serious- mental health disorders and who may have a co-occurring substance-related disorder.
This is a salaried position, 40 hours per week. This position reports directly to the Program Director and also works non-traditional hours / staggered schedule.
