The School Transition Specialist is responsible for the coordination of services between the patients at Garfield Park and the schools.Â This includes assisting the treatment team in obtaining information about the patient's behaviors at the schoolÂ and acquiring copies ofÂ theirÂ Individualized Educational Plans.Â The School Liaison will also function as a resource and advocate for the families when patients require special plans in their current schools.Â They will educate the families on how to obtain the services their child requires to be successful at school.Â Position may also include conducting individual, family and group sessions with all patient populations.
ESSENTIAL JOB FUNCTIONS:

-Ability to quicklyÂ develop working relationships with the GPBH treatment team, patients, families, personnel at schools and community resources
-Fluency in Microsoft Office applications, internet search engines and other computer based databases and applications
-Perform written and typed documentation/patient notes
-Organized with an ability to multitask
-Ability to work independently but also in a team approach setting
