South Franklin Circle is our newest aging-in-place facility located in Chagrin Falls with over 88 acres of lush landscapes. “Crane House”, a 40 Room assisted living/memory care unit seeks full-time Director to be responsible for the overall management of Assisted Living to ensure a safe and comfortable living environment while meeting the unique needs and desires of the members of Assisted Living and their families
In this position you will:
Promote and encourage positive interdepartmental and member relationships.Promote and implement key corporate initiatives that support members (i.e. comfort matters memory care)
Actively partners with other members of the health services team to support member needs.
Leads the team in developing a unique service plan for each member incorporating personal care preferences and care needs that is developed at admission and reviewed quarterly or as member needs changed.
Provide direct supervision and instruction to care partners and nurses including training, scheduling assignments, and evaluating performance, discipline, hiring and firing, etc.
Actively listen to member/family concerns and provide feedback for effective resolution. Provide caring, emotional support to members and families during times of need.
Develop and implement care planning in collaboration with Assisted Living team, members, families, physicians, and other appropriate parties.
Provide education sessions to associates to ensure required continuing education hours are met.
Monitor compliance with federal and state requirements to ensure assisted living is in a state of readiness for periodic state inspections, and take an active role during periodic state survey process
Educate associates on the survey process.
Respond to inquiries and provide tours to prospective members or families. Assess potential members for admission to ensure proper level of care.
Maintain occupancy level goals
Develop and maintain relationships with prospects.
Oversees the completion of all essential admission paperwork when a new member moves to Assisted Living.
Ensure all Member records are accurate and up to date.
Complete Incident/Accident reports and identify and implement appropriate interventions.
Coordinate appointments for members in need of physician services, dental services, etc.
Participates in corporate wide QAPI initiatives and ensures all Member Rights are met.
Work with Program Coordinator on activities during Program Coordinator hours and on evenings and weekends.
Work with nursing staff on medical inventory, order on a monthly basis
Work with care partners on linen inventory, order on a weekly basis
Manages the staffing and schedules inclusive of posting staff schedules every two weeks
May need to be available on evenings and weekends to respond to calls either giving advice or assistance in scheduling during a call off
Be flexible in working schedule to allow for occasional presence during off hour shifts to observe and resolve active challenges and issues
The successful candidate is expected to possess:
Registered nurse with bachelor’s degree preferred
3-5 years of experience working with the elderly population, including experience working with dementia and other chronic conditions.
Management experience. Strong leadership, communications and time management skills
Experience with Microsoft Office is required.
Assisted Living or long term care experience is highly preferred but not required if applicant has commensurate and applicable health care management experience.
