This role partners with the patient and family and serves as an advocate for the patient to support the patient through the continuum of renal care. This role provides necessary resources to help navigate the healthcare system upon referral to Satellite and during periods of transition. This role assists in creating and tracking the patient's choices for dialysis (renal replacement treatment plan) and collaborates with the inter-disciplinary team to guide the patient, family and process to engage all in the patient's care plan.

Serves as the primary Patient Advocate by promoting a patient- and family-centered care environment for optimal decision-making along with their healthcare continuum; acts as a primary point of contact with patient and family
Responsible for driving the Satellite Healthcare culture through values and customer service standards
Conducts a comprehensive resource assessment with the patient and connects them with the needed resources to optimize the patients' experiences
Demonstrates the willingness to help patients; acknowledges and responds to patient's and family's needs
Provides patient with information and support for developing their path on dialysis, tracks patient care progress, and initiates conversations with patient about potential changes to their path on dialysis
Collaborates with other members of the inter-disciplinary team, ensures frequent and timely communication on status of patients to the inter-disciplinary team to promote optimal patient care
Fosters a support network of community resources to better meet the needs of the patients and families
Builds partnerships with local agencies and groups that may assist with dialysis patient care, support, or educational needs
Facilitates Dialysis Modality Options discussions
Provides supportive counseling to patients to explore optimizing treatment plan and implementation
Tracks and monitors patients in the Satellite Healthcare system to ease transitions
Accountable for outstanding customer service to all external and internal customers
Develops and maintains effective relationships through effective and timely communication
Demonstrates initiative and accountability to all patient-related issues in a timely manner and involves the inter-disciplinary team when appropriate
Minimum Qualifications Any combination of education and experience that would likely provide the required knowledge, skills, and abilities as well as possession of any required licenses or certifications is qualifying.
Education: Bachelor's Degree
Experience: Three (3) years directly-related work experience; healthcare patient experience strongly preferred; multilingual candidates preferred
License/Certifications: Current BLS certification (or within 3 months of hire)
