The Community Resource and Referral Center (CRRC) is located in Portland, OR and provides services for Veterans and their families who are homeless or are at risk of becoming homeless. The incumbent is a member of the Patient Aligned Care Team (PACT) and usesprocesses and evidence-based practice to collaborate with the PACT team in developing the patient-driven care plan for life. Theincumbent is responsible for collaborating with services, internal and external to the VA, to facilitate care transition in order to effectively meet the patient's needs and provide patient and family health education with a focus on self-management, prevention and wellness based on the patient's goals. Candidates should possess varied and extensive experience in medical and mental health practices. The position will require significant coordination of care and community outreach to individuals and organizations outside the VA.
Preferred Experience: Experience working with homeless or other vulnerable populations with an emphasis in mental health is desirable.
The Primary Care Staff Physician has responsibility for providing primary care to a defined panel of primary care patients. The incumbent is responsible to contribute to the achievement of quality of care standards, efficiency, process improvement and patient satisfaction within the Primary Care Division, in particular to the incumbent's assigned panel of Primary Care patients. The incumbent is expected to practice in accordance with professional standards, core values, and with a commitment to service excellence.
Work Schedule: The selected candidate will work Monday - Friday, from 7:30 a.m. to 4:00 p.m., with on-call duties required (evenings/weekends), as necessary.
Physical Requirements:?,The work is sedentary, however, there may be some walking, standing, bending, carrying of light items such as papers, books, small parts, or driving an automobile, etc. No special physical demands are required to perform the work.
- U.S. citizenship. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- Degree of doctor of medicine or an equivalent degree resulting from a course of education in allopathic medicine or osteopathic medicine. Degree must have been obtained from an institution whose accreditation was in place for the year in which the course of study was completed. Approved schools are:
(1) Schools of medicine accredited by the Liaison Committee on Medical Education (LCME) for the year in which the degree was granted, or
(2) Schools of osteopathic medicine approved by the Commission on Osteopathic College Accreditation (COCA) of the American Osteopathic Association for the year in which the degree was granted.
(3) For foreign medical graduates not covered in (1) or (2) above, facility officials must verify with the Educational Commission for Foreign Medical Graduates (ECFMG) that the applicant has met requirements for certification, and must obtain a copy of the ECFMG certificate, if claimed by the applicant.
- Residency training or its equivalent, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification. (NOTE: VA Physicians involved in academic training programs may be required to be board certified for faculty status.) Approved residencies are:
(1) Those approved by the accrediting bodies for graduate medical education, the Accreditation Council for Graduate Medical Education (ACGME) or American Osteopathic Association (AOA), in the list published for the year the residency was completed, or
(2) Other residencies or their equivalents which the local Professional Standards Board determines to have provided an applicant with appropriate professional training.
- Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia.
- English language proficiency.
