Salary Range: $50,128 - $75,400
To insure appropriate utilization of health care services and resources and facilitate the continuum of care including discharge to appropriate setting. Daily interaction with the inpatient unit physicians and practitioners to assist in obtaining documentation clarification on a pre-billing basis.
Facilitate coordination of individualized plan of care protocols, pathways and guidelines in collaboration with the attending physician, Social Services; nursing staff, rehab services clinical dietitians, etc.
Assist with patient progression along continuum of care using a case management approach.
Complete comprehensive case management assessment of patient/caregiver.
Assist in development of plan of care for the delivery of appropriate services for patients/caregivers in conjunction with patient, family, patient's physician and other providers.
Arrange/authorize the delivery of appropriate services as agreed to by patient/caregiver.
Monitor the delivery of services for timeliness and appropriateness, adherence to plan, quality, and cost.
Collaborate with other providers (physicians, home care agencies, etc.) to discuss patient/caregiver needs services.
Provide individual patients/caregivers information, instruction, and support. Work with other departments to coordinate services.
Collaborates with the multidisciplinary team serving as a resource to insure appropriate utilization of resources.
Interacts with physicians and specialty practitioners (APRNs, PAs, etc.) to ensure accurate and high quality documentation practices.
Interacts with physicians and other patient care providers regarding documentation policies, procedures, and regulations; obtains clarification of conflicting, ambiguous, or non-specific documentation.
Provides ongoing education to physicians/specialty practitioners regarding physician documentation requirements.
Ability to gather data, compile information, and prepare reports.
Ability to communicate effectively, both orally and in writing.
Interacts with the unit staff and physicians in a pleasant manner to provide continuing feedback on intradepartmental practices and procedures.
Uses the coding staff and Clinical Documentation Specialist staff as a resource when needed.
Maintains knowledge and expertise in electronic software tools Cerner, AllScripts/ECIN, etc.
Maintain current knowledge of Medicaid/Medicare regulations.
Participate in appropriate hospital committee activity.
