ORGANIZATIONAL LOCATION: This position is located within the Great Plains Area, Standing Rock Service Unit (SRSU), under the supervision of the Standing Rock Service Unit's Public Health Nursing (PHN) Department. As the PHN Mental Health Case Manager (PHN CM), the incumbent is responsible for all aspects of Case Management for mental health clients, determining the appropriateness of the client's admission to the PHN Case Management Program, assisting in the development of the Plan of Care, ensuring that the plan is implemented in a timely basis, and identifying patient needs. This position is accountable for promoting an interdependent collaboration with the client, physician/primary care provider, behavioral/mental health care provider, family, and other members of the health care team as well as enhancing the coping and self-management skills of the client and family.
