Providence is calling an experienced Registered Nurse Case Manager to the Palliative Care Unit at Providence Centralia Hospital in Centralia, WA.
We are seeking a compassionate and caring RN to ensure that patients and their families receive quality, cost-effective health care.
The RN Case Manager facilitates safe discharge planning for the patients. They coordinate transportation, locate facilities, help patient's achieve their post-acute care goals, and assist families with locating resources. They also coordinate with insurance companies and ensure CMS rules/regulations are followed.
This is a per diem (on-call), variable shift position.
In this position you will:
Assess and monitor patients, determine eligibility for treatments and ensure the plan of care meets the patient's needs.
Perform comprehensive medical reviews utilizing appropriateness criteria and interpret implications of clinical staus and medical treatment plan with current federal regulations and guidelines.
Ensure that the resource utilization, care delivery, and discharge planning are closely coordinated so as to optimize the patient's stay in the hospital.
Contribute to the clinical performance improvement by providing direction on ways to improve efficiencies, reduce costs, receive appropriate reimbursement, and meet regulatory requirements.
Required qualifications for this position include:
Graduate of an accredited school of nursing
Current Registered Nurse (RN) licensure in Washington State
Three years experience in nursing with prior experience in Utilization Review, Home Health, Quality Management and/or Discharge Planning or an Associate Degree in Nursing.
Demonstrates positive, professional and personal image.
Preferred qualifications for this position include:
Bachelor of Science Degree in Nursing (BSN) or an Associate Degree in Nursing (ADN).
3-5 years of RN experience in an acute care (hospital) setting.
