Ideal candidate will have the ability to complete a comprehensive patient and family assessment to determine priority needs and, in collaboration with the patient's physician and other team members, design and implement an individualized plan of care. Candidate should also be comfortable ensuring that the patient and/or their caregiver are educated on managing their medical condition and comorbidities. Role will be critical in helping the clinic reach our goal to increase the number of completed wellness visits we do annually while maintaining the highest level of patient care.
