Providence is calling a Discharge Planner ( Full Time / Day Shift ) to our Facey Case Management location in Mission Hills, CA.
In this position you will:
Along with RN case manager manage and coordinate all non-clinical duties related to care of members who have complex medicalproblems requiring multidisciplinary intervention.
The purpose is tocoordinate, facilitate, and follow a patient's use of an array of health andsocial services by utilizing community based services to preventinstitutionalization and re-admissions by meeting the member's needs during andafter hospitalization.
Required qualifications for this position include:
Theknowledge and ability listed below may be acquired through different types ofeducation, training and experience. Anexample of a typical way to acquire the qualifying knowledge and ability islisted below:
Two years of currentmedical office or utilization management experience, verified skill setsdefined by department usage.
Basicknowledge of disease processes, ability to recognize problems and to use effectivejudgment in problem solving.
Demonstrate proper phone etiquette.
Strong computer skills including knowledge of GE Centricity/Flowcast, Allscripts.
Knowledge of Microsoft Office (Word, Excel, Outlook).
Preferred qualifications for this position include:
Bilingual in Spanish/English preferred
