Javon Bea Hospital - Rockton Avenue has added a Sub-Acute Unit (LTC/Skilled Nursing). The Sub-Acute Unit offers a variety of services which help meet both the medical and non-medical needs of people with a chronic illness or disability who cannot care for themselves. The care is focused on individualized and coordinated services that promote independence, maximize patients' quality of life, and meet patients' needs over a period of time.

Activity Coordinator, Sub-Acute Unit (LTC / Skilled Nursing), Javon Bea Hospital - Rockton, Days, 48hrs/2wks

Essential duties and Responsibilities

•   Utilizes the activity programming process to ensure all leisure needs of the residents are met in accordance to state and federal guidelines.
•   Works in close collaboration with physicians and other health care personnel in patient evaluation and treatment to further their understanding of significant activities to meet the overall well-being of residents.
•   Provide a specific, planned program of individual and group activities that are aimed at improving, maintaining, or minimizing decline in the resident's functioning status.
•   Implement activities that are multifaceted and reflect each individual resident's needs and be adapted to the resident's capabilities.
•   Specific activities may include: Physical activities, cognitive stimulation, intellectual, educational, spiritual, service activities, sensory stimulation, community involvement, expressive and creative arts/crafts, family involvement. resident council and social activities.

Requirements

Education and Experience 

High school diploma or equivalent required.
 
  

Certifications/Licensure 

2 years of experience in a social or recreational program at a long-term care or skilled nursing facility within the last 5 years
