Javon Bea Hospital - Rockton Avenue has added a Sub-Acute Unit (LTC/Skilled Nursing). The Sub-Acute Unit offers a variety of services which help meet both the medical and non-medical needs of people with a chronic illness or disability who cannot care for themselves. The care is focused on individualized and coordinated services that promote independence, maximize patients' quality of life, and meet patients' needs over a period of time.

Social Worker, Sub-Acute Unit (LTC / Skilled Nursing), Javon Bea Hospital - Rockton Avenue, Days, 64hrs / 2wks
The social worker will work with residents by identifying their psychosocial, mental and emotional needs along with providing, developing, and/or aiding in the access of services to meet those needs. Services will be provided in accordance with State and Federal regulations. The social worker is responsible for fostering a climate that enables residents to maximize their individuality, independence and dignity. This climate shall provide residents with the highest practical level of physical, mental and psychosocial well-being and quality of life. 
The Social Worker works closely with each resident and their loved ones to assist them with the transition into our facility, helping to resolve any issues or problems they may be related to discharge back home or to the community, and providing therapeutic approaches to working through problems.  The Social Worker is a member of the interdisciplinary team that is essential in providing a comprehensive approach to resident care in a person-centered environment.

The Social Worker oversees the facilities grievance process and is the designated Grievance Officer, they are responsible for receiving, investigating and tracking grievances through to the conclusion, including leading necessary investigations, maintaining the confidentiality of all information associated with grievances, communicating with residents/resident representative throughout the process of resolution, coordinating with other staff including the Administrator, Assistant Administrator and Director of Nursing as needed.

Essential duties and Responsibilities
•Provides medical related social services so that each resident may attain or maintain the highest practical level of mental, physical, and psychological well-being.
•Ensures timely documentation on each resident in the facility as required.
•Begins discharge planning on day one. Works with the interdisciplinary team to identify discharge planning needs, acting as a resource and referral source for the complex inpatient and post discharge needs implementing a safe and appropriate discharge plan.
•Assesses and assists with the complex psychosocial and educational needs of each resident, by arranging services in order to meet identified resident and family needs. 
•Coordinates and facilitates patient care activities to improve continuity of care across the health care continuum, to promote appropriate utilization of resources and to contribute to improved patient satisfaction and outcomes. 
•Completes PASARR on every admission and submits when indicated.
•Acts as a resident advocate, as a resource to residents/families, staff and physicians. 
•Provides interpretation of psychosocial status/needs to the staff.  
•Assists families and residents with financial questions and referrals
•Coordinates with the resident and family/resident representative to review policies on Advanced Directives upon admission.  Assists residents in completing advance directives and DNR status; educates residents about resources and options available; and documents all activities in a professional, timely and concise manner. 
•Arranges for appropriate services to meet the needs of the residents i.e. psych, dental, vision, etc
•Ensures State and Federal regulations related to all social services needs are followed and ensures organizational compliance with those that are related to ethics, patient rights and discharge planning.
•Participates, collects and analyzes psychotropic medications data along with the Director of Nursing  providing a monthly report to the Administrator and QAPI Team.  Participates in Monthly QAPI Meetings.
•Coordinates referrals and authorizations to home health care, DME, transfers to other SNF’s, other Rehabs and follow up physician appointments.
•Organizes, conducts, and participates in resident’s plan of care conference which includes invitation to attend the meeting to care staff, resident, and families.  
•Identifies and reports all allegations of mistreatment/Abuse to the Administrator immediately per Mercyhealth SNF Abuse Policy and Prevention Protocols.  Acts as a liaison with county officials for residents involved with protective services; aging, and mental health services; legal guardianship actions; and economic support.  Promotes and monitors resident and with department services.  
•Maintains a database of area resources, actively seeks new resources, and shares this  information with the inter-disciplinary team 
•Completes  social services portion of the MDS and CAA’s, establishes resident care plans, updating as needed and documents episodic issues in resident’s medical record.
•Reviews and completes admitting paperwork as assigned with residents/families and/or their representatives at the time of admission.
•Completes the Social Services portion of the MDS assessments, resident demographics, and histories timely and accurately. 
•Assists with implementing and providing communications with palliative care and  hospice services. 
•Participates in resident/facility rounds. 
•Adheres to and enforces safety procedures 
•Reports medical concerns or observations.
•Arranges necessary services for counseling or support services. 
•Assists with psychiatric medication management. 
•Initiates one on one conversation, routine visits with residents, and facilitates difficult conversations with residents and families.



Requirements

EDUCATION & EXPERIENCE  

Bachelor's degree from four-year College or university in Social Work, Psychology or related field
