SEM Haven is a family, which includes elders and staff, as well as elder families and staff families. The final goal is to provide support for each of its members. Your role as a staff member is to provide the elders of SEM Haven with the support, friendship, and most importantly, kindness, so they may live as independently as they wish to in their homes. The most important function of your position is that you treat everyone you encounter as a person deserving respect. No person is any more or less important than any other and all should be treated in this manner.
As a member of a family team you have the additional responsibility to perform tasks as necessary to maintain life on the household. These tasks will be within your knowledge and training i.e. assist with food preparation/serving; picking up clutter, removing trash, cleaning spills; transporting elders; attending household meetings/learning circles and fulfilling simple requests for elders.
STNA:
Participates in and receives assignment and nursing report; gives feedback to nurse on elders needs or changes in condition; follows plan of care and assignment sheet; attends elders care conferences
Explains procedures to elders before administering and plans care delivery to honor elders preferences
Assists elders with care including bathing, dressing, grooming, nail care, oral hygiene and use of adaptive devices according to plan of care and elders wishes
Assists with lifting, positioning, turning and transferring elders into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc., in keeping with specific safety needs
Carries-out Restorative Programs including range of motion, ambulation and bowel and bladder training as planned and per instruction of the nurse; accurately records delivery of services and progress or problems
Ensures call lights are within reach of elders and answers call lights promptly; Ensures that elders who are unable to call for help are checked frequently
Uses proper technique in making/changing beds (occupied and unoccupied); uses proper linen handling techniques for infection control.
Assists elders in preparation for and transporting to appointments, activities and social programs; leads activities as assigned.
Accurately measures and records height, weight, I&0, appetite, vital signs and others as instructed.
Completes appropriate charge and inventory forms; uses supplies economically; reports needs
Requirements:
Education/Experience: State Tested Nursing Assistant in the state of Ohio, on the registry or eligible for testing.
LPN or RN:
The Charge Nurse works cooperatively and respectfully with elders, coworkers, families, visitors and volunteers to create a positive and pleasant environment in the home.
He or she delivers nursing care to elders according to facility policies and procedures, quality improvement guidelines and local, state and federal regulations.
The Charge Nurse actively promotes the elder's opportunities for engaging in normal life pursuits and improving and/or maintaining functioning levels.
The Charge Nurse is a participant in elder-centered care and team-based management and is involved in daily life activities in his or her assigned household(s).

Homemaker (Dietary Aide):
Cleans & disinfects all surfaces within assigned area using approved chemicals; understands how to use MSDS sheets; keeps chemicals in locked areas
Replenishes supply of towels, toilet tissue and soap in kitchen
Wears gloves during cleaning; changes and washes hands after each room; cleans isolation rooms using proper personal protective equipment
Serves meals; offers supplements and substitutes
Sets dining room tables for meals; ensures they present a neat and clean appearance.
Knowledgeable about food safety, kitchen sanitation, portion size and correct serving utensils.
Knowledgeable about meal consistency and ensures elders are served properly as planned (puree, ground, regular, thickened liquids).
Follows planned menus; discusses with dietetic technician any problems with meal consumption, likes or dislikes, and special preferences of the elders.
Cleans the kitchen & dining room (tables, chairs & floor), buses, cleans & stores dishes, puts away all supplies and removes trash.
Stocks kitchen with food and supplies needed for the elders; labels and dates all food stored in the kitchen, including dates delivered and dates opened.
Practices the rules of sanitation at all times when working in food service/dining areas.
Is knowledgeable about the potential for food borne illness.
Uses proper techniques for handling food items.
Ensures that safety and sanitation of the household is a priority.
Reports any equipment or sanitation issues to the appropriate person.
Keeps accurate records of food serving temp, refrigerator temp, sanitation levels and cleaning schedules.

Requirements:
EDUCATION/EXPERIENCE: Sufficient education to demonstrate functional literacy.
Food service experience preferred.
Untested Aides In Assisted Living:
Participates in and receives resident assignment and nursing report; gives feedback to nurse regarding resident needs
Assists residents with activities of daily living including bathing, grooming, dressing, hygiene, incontinence care and use of adaptive devices
Explains procedures to residents before administering and plans care delivery to honor resident preferences
Uses proper technique in making/changing beds
Handles linens according to infection control guidelines
Assists residents in preparation for and transporting to appointments, activities and social programs
Assists with transferring, positioning and transporting residents into and out of beds, chairs, shower and wheelchairs in keeping with specific resident safety needs
Assists residents in transferring and ambulation with or without self-help devices as instructed
Reports all changes in resident's condition to nurse as soon as practical
Provides post-mortem care as instructed
Keeps residents' water pitchers clean and filled with fresh water and within easy reach of residents
Communicates signs symptoms of change and residents concerns to appropriate staff
Makes new residents comfortable and orients to the home
Inventories and stores residents' personal possessions as instructed
Establishes and maintains good public relations with residents, families and other facility guests, shows empathy toward residents
Maintains cleanliness of resident rooms and common areas
Performs routine housekeeping duties (trash, bed making and laundry)
Cleans & disinfects all surfaces within assigned area using approved chemicals
Does whole room thorough cleaning and bed scrubs according to schedule
Cleans & disinfects all restrooms daily and as needed, including floors
Participates in planning and carrying out household activities, including one-on-one resident visits; documents resident participation.
Participates in special events planned for the household and SEM Haven.

Cooks:
Prepares meals according to menus and production sheets; monitors elders acceptance and preferences
Works with dietitian & culinary director to make changes according to elders preferences
Uses nutritionally equivalent substitutes when needed; records menu changes
Assists with special events as requested
Maintains standards for food preparation and quality of food service
Ensures proper portion control and minimizes waste; always maintains sanitation and safety standards in food preparation
Ensures food and supplies are stored properly
Prepares food according to established recipe and production sheets
Tests food by smell, taste and sight to determine if it is properly cooked and palatable.
Operates a variety of kitchen utensils and equipment, such as blenders, mixers, grinders and slicers. Bakes, roasts, broils, and steams food items to prepare for consumption; prepares food items according to HACCP standards and spreadsheets.
Washes, peels, cuts, and shreds vegetables and fruits to prepare for use
Labels and dates all food stored in the kitchen, including dates delivered and dates opened.
Takes and records pre and post serving temperatures of food and beverages; serves food at appropriate temperature
Requirements:
Education/Experience: Experience in cooking in a food service setting, preferably in a health care facility; certification in food safety desired.
